This kasakoa is very interesting. From what i learned this kasakoa comes from the idea that the person your talking about is close to you like a liver. We say this because if you look at an animal, like the chicken, you can see how close the bile sack is to the liver and in order to remove the liver (to eat it) you must be very careful with the way you cut around the bile so that it will not break and destroy the liver.

Because this person is a close friend you don't want to be reckless and destroy the friendship (i.e. The liver).
